Post-class survey

When learners are submitted on a class roster, they receive an email from ICAgile inviting them to complete a survey in order to claim their certification. The survey includes the following questions:

1. How was this course delivered? (In-Person / Remotely / Both In-Person and Remotely)
2. How many students attended the class? 
3. How applicable was the material to you in your current role? (1-5)
4. How likely are you to recommend this course to a colleague?* (0 - 10)
5. How likely are you to recommend [Instructor Name] to a colleague?* (0-10)
6. How knowledgeable was the instructor of the subject matter? (1-5)
7. What suggestions do you have to improve the course content? 
8. What additional feedback do you have for the instructor?

*Questions 4 and 5 calculate course and instructor Net Promoter Scores.
